Dear Program Participants,

In this package you will find the following information:

• Registration Form (one) for Modules # 1 through 6 of NUHS

• A synopsis of the Modules in the order of their presentation

• Application for Credentialing from the American Chiropractic Physician Credentialing Center, which is one of the requirements for advanced practice. Please begin this process as it takes a bit of time.

As soon as I have information on the textbooks required, I will be forwarding that out to each of you through our e-mail list.

For your information, Modules # 1 through 3 must be taken in order. So, if you have to miss # 1, you cannot take #s 2 or 3 and, if you miss # 2, you cannot take # 3 and so forth. If you have to miss one of modules # 4 through 6, you can take the next module and make up the one you missed at a later time when the courses are offered again, which we hope to do in 2009.

Please be sure to get your registration forms to me, as early as possible, so that I have an accurate count for each Module. If there are those of you that wish to spread the payments out, I have no problem with accepting your check or credit card closer to the actual dates but still request that you register in advance and let me know when I can expect to receive your payment.
