Modules #2 and #3 ~~ Information & Registration

Dear Program Participants,

In this package you will find the following information:

• An explanation of HB 275 (now passed into law) and, generally, what will be required to obtain a Chiropractic Advanced Practice certification written by Dr. George Simmons

• Registration Form for Module #2, July 12 & 13, 2008

• Registration Form for Module #3, August 16 & 17, 2008

• Application for Credentialing from the American Chiropractic Physician Credentialing Center, which is one of the requirements for advanced practice. Please begin this process as it takes a bit of time.

Please be sure to get your registration forms to me, as early as possible, so that I have an accurate count for each Module. If there are those of you that wish to spread the payments out, I have no problem with accepting your check or credit card closer to the actual dates but still request that you register in advance and let me know when I can expect to receive your payment.
